The short answer is no—but there’s an important catch that affects up to half of all patients. Understanding what can happen after cataract surgery helps you maintain the clear vision you worked hard to restore.
During cataract surgery, your eye surgeon removes the clouded natural lens and replaces it with a clear artificial lens called an intraocular lens (IOL). Because the original lens is gone, a true cataract cannot redevelop. The artificial lens stays clear permanently.

This condition, called posterior capsule opacification (PCO), occurs when the thin membrane that holds your new lens in place becomes cloudy. The good news? PCO is easily treatable with a quick, painless laser procedure called YAG laser capsulotomy. This five-minute outpatient treatment uses focused laser energy to create a small opening in the cloudy capsule, immediately restoring clear vision. No incisions, no anesthesia beyond numbing drops, and no recovery time required.
Most patients notice improved vision within hours of the procedure. Unlike cataract surgery itself, YAG capsulotomy is a one-time fix—once the capsule is treated, PCO cannot return in that eye.
While anyone who has had cataract surgery can develop PCO, certain factors increase your risk. Younger patients tend to develop PCO more frequently than older adults. People with diabetes, uveitis, or other inflammatory eye conditions also face higher rates of capsule clouding.
