The condition typically emerges during puberty, though cases have been documented in younger children and adults. Women appear to be diagnosed more frequently than men, though the small number of cases makes definitive patterns difficult to establish.
Diagnosis requires a water challenge test, where a doctor applies a water compress to the patient’s skin for 30 minutes and monitors for hive development. This test helps distinguish aquagenic urticaria from other forms of physical urticaria triggered by pressure, temperature changes, or exercise.
Treatment options remain limited and focus on symptom management rather than cure. Antihistamines taken before water exposure can reduce the severity of reactions for some patients. Others find relief through phototherapy treatments or barrier creams applied before bathing.
“Many patients develop coping strategies like taking very brief showers in lukewarm water or limiting water contact to essential activities only,” says Dr. Feldweg.
The psychological toll of living with such a rare condition extends beyond physical discomfort. Many sufferers report feeling isolated and struggling to explain their condition to others who find it difficult to believe that something as universal as water could cause such distress.
