A blood test you may have never heard of could hold important clues about inflammation happening inside your body. The erythrocyte sedimentation rate (ESR) test is a simple procedure that helps doctors detect and monitor inflammatory conditions affecting people over 40.

This decades-old diagnostic tool measures how quickly red blood cells settle to the bottom of a test tube. When inflammation is present in your body, proteins in your blood cause red blood cells to clump together and fall faster than normal.

“The ESR test is particularly valuable because it gives us a broad snapshot of inflammatory activity in the body,” says Dr. Margaret Chen, a rheumatologist at Johns Hopkins Medicine. “While it doesn’t tell us the specific cause, it alerts us that something requires further investigation.”

The test itself is straightforward. A healthcare provider draws blood from your arm and places it in a tall, thin tube. After one hour, they measure the distance red blood cells have fallen. Normal results typically range from 0-22 mm/hr for men and 0-29 mm/hr for women, though these ranges increase slightly with age.

High ESR readings can indicate various health concerns. Infections, whether bacterial or viral, commonly elevate sedimentation rates as your immune system responds. Autoimmune diseases like rheumatoid arthritis and lupus also produce elevated results, making ESR a useful monitoring tool for these chronic conditions.

Cancer, kidney disease, and thyroid disorders may similarly cause increased ESR values. Even pregnancy and certain medications can temporarily raise your numbers. This wide range of potential causes is why doctors never rely on ESR results alone.

“We always interpret ESR in context with a patient’s symptoms, medical history, and other lab work,” explains Dr. Robert Martinez, an internist specializing in preventive care for adults over 50. “An elevated ESR is a red flag that guides us toward additional testing, not a definitive diagnosis.”

Low ESR results are less common and typically not concerning. They may occur with conditions affecting red blood cell shape or number, such as sickle cell anemia or polycythemia. Some medications, including corticosteroids and aspirin, can also lower ESR readings.

For those managing chronic inflammatory conditions, regular ESR testing helps track disease activity and treatment effectiveness. A decreasing ESR often signals that medications are working, while rising numbers may indicate a flare-up requiring intervention.

The test has limitations, however. Results can be affected by factors unrelated to inflammation, including age, gender, obesity, and even the time of day. Some people with active inflammatory diseases maintain normal ESR levels, while others have persistently elevated readings without any identifiable health problem.

“ESR is one piece of the diagnostic puzzle,” notes Dr. Chen. “We combine it with C-reactive protein tests, complete blood counts, and imaging studies to get the full picture of what’s happening in a patient’s body.”

If your doctor orders an ESR test, no special preparation is required. You can eat and drink normally beforehand, and the blood draw takes just a few minutes. Results are typically available within a day.

Understanding your ESR results empowers you to have informed conversations with your healthcare provider about your health status. While the test itself is simple, the information it provides can be instrumental in detecting conditions early when treatment is most effective.

For adults over 40, maintaining awareness of inflammatory markers becomes increasingly important. Chronic inflammation has been linked to heart disease, diabetes, and accelerated aging. Regular health screenings that include inflammatory markers help catch problems before they become serious.
