An EKG works by measuring the electrical signals that make your heart beat. Every time your heart contracts to pump blood, an electrical impulse travels through the heart muscle. The test can detect several serious conditions. Arrhythmias—irregular heart rhythms—show up clearly on an EKG tracing. The test can also reveal evidence of a previous heart attack, even if you didn’t know you had one. Some heart attacks cause minimal symptoms, particularly in women and people with diabetes.

“We call them ‘silent heart attacks,’ and they’re more common than most people realize,” Dr. Mitchell notes. “An EKG can show scarring in the heart muscle that indicates damage occurred at some point, which is critical information for preventing future problems.”

An EKG can also detect signs of coronary artery disease, where plaque buildup restricts blood flow to the heart. When the heart muscle isn’t getting enough oxygen, it produces distinctive changes in the electrical patterns that a trained physician can recognize.

Structural problems with the heart also leave their mark on an EKG. An enlarged heart—often caused by high blood pressure or heart valve disease—changes the electrical patterns in predictable ways. Inflammation of the heart muscle or the sac surrounding it shows characteristic changes as well.

While a resting EKG provides valuable information, some heart problems only appear during physical exertion. That’s why doctors sometimes order a stress test, which involves recording an EKG while you walk on a treadmill or ride a stationary bike. This helps identify blockages that might not cause symptoms when you’re at rest but could be dangerous during exercise.

For intermittent symptoms—heart palpitations that come and go, for example—your doctor might recommend a Holter monitor or event recorder. These portable devices record your heart’s electrical activity continuously for 24 to 48 hours, or longer in some cases, capturing abnormalities that might not occur during a brief office visit.

Interpreting an EKG requires specialized training. The printout shows multiple wavy lines representing electrical activity from different angles around your heart. Doctors look at the size, shape, and timing of these waves to identify problems.

“Every bump and valley tells us something,” Dr. Mitchell explains. “The P wave shows atrial contraction, the QRS complex represents ventricular contraction, and the T wave shows recovery. Changes in any of these components can indicate specific problems.”

One of the EKG’s greatest strengths is its accessibility. Unlike many advanced cardiac tests, EKGs are available in virtually every medical setting—from emergency rooms to primary care offices to ambulances. The machines are relatively inexpensive, and the test is covered by Medicare and most insurance plans when medically necessary.

However, an EKG does have limitations. A normal EKG doesn’t guarantee you don’t have heart disease—it only shows what’s happening during those few minutes of recording. Some blockages or rhythm problems might not be apparent at rest, which is why doctors often combine EKGs with other tests like echocardiograms or cardiac CT scans for a complete picture.

Preparation for an EKG is minimal. You don’t need to fast or avoid medications (unless specifically instructed otherwise). Wearing a two-piece outfit makes electrode placement easier. Men with chest hair may need small areas shaved for better electrode contact.

Let your doctor know about all medications you’re taking, as some can affect heart rhythm. Also mention if you have a pacemaker or implantable defibrillator—these devices show up on the EKG and your doctor needs to account for them when interpreting the results.

After the test, you can resume normal activities immediately. Your doctor will review the results and discuss any findings with you. If abnormalities appear, additional testing might be needed to determine the exact problem and appropriate treatment.

For adults committed to aging well, the EKG represents preventive medicine at its most practical. Heart disease remains the leading cause of death for Americans over 40, but it’s also one of the most treatable conditions when caught early.
