FDA Lifts Black Box Warning on Menopause Hormone Therapy

The Food and Drug Administration is updating menopause hormone therapy guidance by removing the longstanding black box safety warning. Women are now encouraged to discuss the risks and benefits of hormone therapy with their healthcare providers, especially if they consider treatment early in menopause.
This surprising change follows research showing lower risks for women who start therapy closer to symptom onset, with benefits like improved heart and brain health.
“The alarming black box warning in general has led to underutilization of HRT among many women who are appropriate candidates and would benefit from treatment,” says JoAnn Manson, MD, DrPH, of Harvard Medical School.
Instead of a blanket warning, the FDA emphasizes informed conversations tailored to each woman’s history and needs.
JoAnn Pinkerton, MD, from the University of Virginia, praises the decision. “Removing the scary boxed warning from local vaginal estrogen is a victory for women’s health and for those who care for and about menopausal women,” she says.
Oral hormone treatments still require careful discussion, as benefits and risks vary based on age, timing, and personal health history. Dr. Stephanie Faubion of the Menopause Society notes, “The benefits are greater and the risks lower for younger women initiating hormone therapy for symptom management, closer to the menopause transition.”
Dr. Manson encourages open dialogue: “Instead of being scared away by a one-size-fits-all black box warning, they can have a more nuanced conversation and make an informed decision based on their individual circumstances.”
